Abstract
Objective: To evaluate the use of Doppler ultrasound for the uterine arteries late in the first trimester for prediction of pre-eclampsia few months before the onset of manifestations.
Design: Prospective cohort study.
Participants and Methods: Five hundred singleton preg-nant women between 11-14 weeks gestation were subjected to uterine artery Doppler Pulsatility index (PI) measurement and followed up till delivery. Those who developed pre-eclampsia were compared to those who remained normotensive, and pre-eclampsia was further classified according to the time of occurrence into early and late.
Results: 484 women completed their follow-up. Sixty-three women developed hypertension and there was a statis-tically significant difference in the uterine artery PI measured in the first trimester between those who developed early pre-eclampsia and those who remained normotensive (p=0.0251). Sensitivity was calculated as 54.6% and specificity 88.4% with overall accuracy of 87.6%.
Conclusion: Doppler ultrasound measurement of uterine artery late in the first trimester has reasonable accuracy in prediction of those at risk to develop early pre-eclampsia, however, its role and incorporation in routine antenatal care services as a screening tool for pre-eclampsia is yet to be established.
