Value of Multisticx 10 SG Reagent Strips in the Early Diagnosis of Spontaneous Bacterial Peritonitits, MAHA HASABALLA, MOATAZ SEYAM and MOHAMAD MAHMOUD

 

Abstract
Background: Spontaneous bacterial peritonitis is a fatal complication of cirrhotic ascites. PMNL count in ascitic fluid is the gold standard for diagnosis of SBP; however it may be delayed for several hours in the laboratory department or may be not available in some centers. The aim of this work was to evaluate the effectiveness of the urinary strips Multisticx SG 10 for rapid diagnosis of SBP.
Patients and Methods: Our study included 100 patients with SBP and 30 patients with cirrhotic ascites served as control. All patients were subjected to full clinical evaluation, routine labs, and abdominal ultrasound together with Ultra-sound guided aspiration of the ascitic fluid. Biochemical and bacteriological analysis of the ascitic fluid was done including PMNL in ascitic fluid. Urine dipstick Multisticx SG 10 was used as screening test for diagnosis of SBP in the ascitic fluid. The sensitivity, specificity, positive and negative predictive values of two different colorimetric cut off scales (>!2 and >3) were calculated.
Results: The sensitivity, specificity, positive and negative predictive values at >!2 and >3+ cut off scale were 98%, 40%, 84.4%, 85.7% respectively and 97%, 56%, 88.1%, 85% re-spectively.
Conclusion: Dipstick test can be used as a rapid test for screening of SBP. The higher cut off colorimetric scale has better specificity and positive predictive value with slight decrease in the sensitivity.
