Periprocedural and One Year Out-Come of Carotid Angioplasty,AMR A. YOUSSEF, MOHAMED ABDELGHANY, NADIA M. SALEIM and BERNHARD REIMERS
Abstract
Aim of the Work: To assess the periprocedural and one year longterm follow-up for the results of carotid angioplasty and its safety in patients with carotid atherosclerotic disease.
Patients and Methods: This study was conducted on 102 patients (107 procedures), 43 patients (42%) with symptomatic carotid stenosis (³50% carotid lesion) and 59 patients (48%) with asymptomatic carotid critical stenosis (³70% carotid lesion). Carotid angioplasty and stenting were done in all cases. Clinical and carotid Duplex follow-up were done after one month, 3 months, 6 months and with phone call for one year.
Results: The mean age ±SD of our patients was 72.17±8.62 years. All procedures (100%) were carried out with cerebral protection. Procedural complications were, hypotension in 3 patients (2.9%), access site complications occurred in 4 patients (3.9%) (dissection of the common femoral artery in one patient and hematoma in three patients), spasm of the internal carotid artery in 6 patients (5.9%), dissection of the common carotid artery in one patient. Neurological complications occurred in 5 patients (4.9%), major stroke in one patient, minor stroke in two and transient ischemic attacks in two. The incidence of neurological complications was significantly higher in symptomatic patients than asymptomatic patients. One month follow-up showed no neurological complications, after 6 months follow-up one patient developed minor stroke, another patient developed major non fatal stroke and restenosis oc-curred in 3 patients. At one year follow-up, no neurological complication had occurred.
Conclusions: Carotid artery stenting is a safe and effective procedure for the treatment of carotid artery stenosis especially in high risk patients.
