Abstract
Objective: Is to study role of instrumental vaginal delivery in modern obstetrics and its benefits to lower the rate of caesarean section together with its hazards on mother and fetus.
Patients and Methods: It was a retrospective analysis of 1995 cases conducted from September 2014 till end of February 2015 at the Department of Obstetrics and Gynecology of Benha University Hospital & Kaphr Shukr Hospital. Data were collected from parturient sheet by observation during labor. Collected data were tabulated and analyzed using SPSS version 16 soft ware.
Results: Incidence of ventouse deliveries was 1.0%, forceps delivery 0.0% and caesarean section 53.8%. Common indications of ventouse application were maternal exhaustion, fetal distress and heart disease. 30% of cases had ventouse related maternal complications. Two cases of vaginal tears, two cases of perineal tears and one case of post partum hemorrhage. Failure of ventouse, requiring caesaren section was observed in 20% of cases. 20% of cases had ventouse related fetal complications. One case had respiratory distress syndrome, three cases had scalp injuries. Overall neonatal outcome was satisfactory.
Conclusion: Vacuum extractor must be used cautiously and when is indicated with experienced hands. Implementation of proper active management of labor to avoid false diagnosis of failure of progress. Also we recommend availability of anaesthesia all over the day in public hospitals for proper management of cases needing caesarean section intervention.
