Abstract
Objectives: To assess outcomes of laparoscopic repair of different types of ventral hernias versus open surgical repair early in post-operative period as well as after one year.
Patients and Methods: This is a prospective randomizing study on forty patients who were divided into two groups: Group (A); twenty patients were subjected to open tension free mesh on lay repair method and Group (B); twenty patients were subjected to repair via laparoscopic technique.
Results: There was no significant statistical difference regarding patient demographics. Operative time (hours); at Group (A) was (2.1±0.2) Vs Group (B) (1.3±0.4) with p< 0.001. Hospital stay (days) at Group (A) was (4.7±0.8) while at Group (B) was (1.9±1.5) with p<0.001. Post-operative pain killer doses in open repair were more than laparoscopic repair; (12.3±1.9) Vs (5.4±2.1) respectively with p<0.001. Regarding post-operative complications; only 1 patient in Group (B) during laparoscopic repair had small bowel injury with con-version to open procedure, also post-operative Seroma was more with laparoscopy and observed in 5 cases (12.5%) but other complications like hematoma (5% Vs 2.5%), mesh infection (5% Vs 0%) and hyper trophic scar were developed more in open repair with p>0.05.
Conclusion: Laparoscopic ventral hernia repair is a safe and excellent. Advantages of the laparoscopic approach (i.e. a short hospital stay and brief convalescence) also reduced morbidity and earlier recovery. Also allows identification and repair of previously clinically silent defects. So laparoscopic repair is used with increasing frequency in everyday surgical practice.
