Abstract
Objective: To assess the functional, clinical and radiological outcomes in patients with marginal impaction as part of a simple or complex fracture, and to identify factors that predict outcome.
Design: Prospective study.
Patients: 25 patients with acetabular fractures with marginal impaction.
Intervention: All acetabular fractures with marginal impaction that had been identified by СT were treated operatively and followed-up over 2 years.
Main Outcome Measurement: Clinical outcome was assessed using the Merle d'Aubigne score. Radiographs were evaluated for postoperative reduction, arthritis grade and development of heterotopic ossification.
Results: The mean patient age was 36 years. The quality of the reduction, according to Matta's criteria, was recorded as anatomical in 18 hips (72%), imperfect in 6(24%) and one hip was recorded as poor (4%) post-operatively. At the final follow-up, radiologically 17 hips (68%) were graded as excellent, 6 (24%) as good, and two (8%) as poor. The mean follow-up was 12 months. The modified Merle D'Aubingne and postel score system showed excellent results in 4 patients, very good 12, good 6, fair 1, and poor inl. No significant correlation is found between the following factors: Age, type of the fracture and time to surgery and the radiological outcome; follow-up outcome and the Score. There is reversal correlation bet presence of comminution and radiological outcome with no significant difference in follow-up outcome and the Score, Significant correlation is found between the radiological outcome and both the follow-up outcome and the functional score (p-value<0.001).
