Impact of Uncontrolled Diabetes Mellitus on the Surgical Outcome of Lumbar Disc Surgery, ASHRAF EL-SALAWY

 

Abstract
Objective: To detect the effect of uncontrolled D.M in patients with sciatica secondry to lateral lumbar disc and were candidate for surgery, on the surgical outcome regarding improvement of the preoperative lumbar discogenic radicul-opathy they had.
Setting: In Kasr El-Ainy Hospitals, El-Manyal Specialized Hospital.
Methods and Subjects: A comparative study was carried on 40 diabetes type 2 patients suffering from refractory lumbar discogenic sciatica and underwent surgical discectomy to relief radiculopathy. They were divided into two groups: Group A with uncontrolled DM as proven by labs in the 3 months prior to surgery. Group B: With good control of DM in the same period.
Results: In Group A: 12 patients out of 20 i.e 60% had shown no or limited improvement in radiculopathy despite the proper surgical discectomy to decompress the affected lumbar nerve root, while in Group B: 5 patients only out of the 20 of this group i.e 25% had shown no or limited improve-ment as described above. So, we can notice a significant difference between the two diabetic groups regarding the surgical outcome results regarding improvement of lumbar radiculopathy.
Conclusion: Uncontrolled DM at least 3 months prior to lumbar discectomy surgery was significantly associated with poor outcome in surgical outcome regarding radiculopathy inspite of proper surgical decompression of the affected lumbar nerve root.
