Comparative Study between Haunt Laurance Pouch and A New Modified Technique of Jejunal Pouch (Gastric Substitutes after Total Gastrectomy), MAHMOUD ABD EL-AZIZ, ADEL DENEWER, SHEREIF KOTB, USAMA HUSSEIN and SAMEH ROSHDY
Abstract
Introduction: Total gastrectomy, is associated with many early and late postoperative complications such as malabsorp-tion, reflux oesophagitis, dumping syndrome and weight loss. To prevent these post-gastrectomy complications, more than fifty reconstructive procedures after total gastrectomy in patients with gastric cancer are applied. However, these reconstruction procedures have limitations for preserving the reservoir function and digestion after total gastrectomy, which result in some common problems such as dietary restriction, early satiety, postprandial fullness, vomiting, heartburn, and diarrhea.
Patients and Methods: Thirty patients with gastric carci-noma who underwent total radical gastrectomy were classified into 2 groups, the 1st group underwent reconstruction by (a new modified technique of jejunal pouch), the 2nd group underwent reconstruction by (Haunt Laurance pouch) with comparison of the two groups as regard the operative outcome and as regard the postgastrectomy symptoms on the follow-up.
Results: Statistical analysis showed that there was no difference as regard the operative outcome between the 2 groups but there were improvement in the postgastrectomy symptoms in the 1st group.
Conclusion: In our study, there were improvement in the post gastrectomy symptoms such as dumping, heart burn, vomiting, early satiety in the 1st group who was reconstructed by a new modified technique of jejunal pouch. But to prove which technique is the best for reconstruction after total gastrectomy, it needs more investigations and studies.
