Effect of Renin Angiotensin System Blockade on Renal Dysfunction in Experimentally Induced Diabetic Nephropathy, AHMED A. ABD AL-FATTAH and MAHMOUD E. SALAMA

 

Abstract
Background: Diabetes mellitus is considered as the un-derlying cause of many renal complications. Targeting Renin Angiotensin System (RAS) is the most validated strategy for slowing the progression of diabetic complications.
Aim: The aim of this work was to study the effect of renin angiotensin system blockade on the progression of diabetic complications in the streptozotocin (STZ) induced diabetic rat model.
Materials and Methods: This study was carried on 50 male waster rats. That the rats were divided into 5 groups (10 rats per each). Group I: (Control group). Group II: (Diabetic group). Group III: Diabetic treated with ACE inhibitor (ram-ipril) (0.9mg/kg/d) orally for 60 days. Group IV: Diabetic treated with Angiotensin II receptor antagonists (AT 1) blocker (telmisartan) (3.5mg/kg/d) orally for 60 days. Group V: Diabetic treated with both ACE inhibitor and AT 1 blocker. Diabetic state in experimental rats was induced by STZ 55 mg/Kg intraperitoneally.
Results: Serum markers of kidney damage were signifi-cantly elevated by diabetes with significant improvement by Angiotensin-Converting Enzyme Inhibitor (ACEI), Angiotensin Receptor Blocker (ARB) and both. Combination had higher beneficial effect in Diabetic Nephropathy (DN) especially creatinine clearance, urinary protein and urinary albumin/ creatinine ratio. Biochemical changes were corroborated by histopathological findings.
Conclusion: Combination of ACEI and ARB showed improvement of some biochemical parameters and histopatho-logical changes in diabetic nephropathy. This is of great value to prove the pivotal role of the local RAS in the pathogenesis of diabetic renal complications.
