Abstract
Background: Cardiovascular complications are common in patients with Chronic kidney disease (CKD) and are the leading causes of death in these patients. In this study, we aimed to investigate the relation between atherosclerosis and Left Ventricular Hypertrophy (LVH) and left ventricular systolic and diastolic functions in CKD patients.
Methods: This study included 50 CKD patients. We measured biochemical markers, creatinine, urea, albumin, hemoglobin, fasting blood glucose and lipid profile. Twelve lead ECG, transthoracic echocardiography, carotid and femoral arterial Duplex were performed.
Results: Prevalence of atherosclerosis was 46% in all CKD patients and there was no significant difference between patients on hemodialysis and patients not on dialysis (35% vs. 53.3%, p=0.203). Atherosclerotic patients had higher Left Ventricular Mass Index (LVMI) (151.05±38.68 vs. 114.49±  42.23 g/m2, p=0.001) and higher prevalence of left ventricular hypertrophy (91.3% vs. 48.1%, p=0.001) than non-athero-sclerotic patients. Although atherosclerotic patients had higher prevalence of left ventricular systolic dysfunction (39.1 % vs. 22.20%, p=0.193) and diastolic dysfunction (78.3% vs. 55.6%, p=0.091), yet it did not reach statistically significant difference from non-atherosclerotic patients.
Conclusions: Atherosclerosis affected about half of CKD patients. Patients with CKD and atherosclerosis have increased LVMI. The added CV risk by increase LVMI and LVH in patients with CKD and atherosclerosis warrants more aggres-sive risk factors control and closer follow-up.
