Efficacy and Safety of Basal Plus Regimen in Type 2 Diabetes Mellitus Patients in Egypt, NABIL N. REZK

 

 Abstract
Background and Objectives: Basal insulin provides an effective method for initiating insulin therapy in people with Type 2 Diabetes ('1'2DМ), resulting in significant improvements in glycemic control. The addition of a single prandial insulin injection at the largest meal is well tolerated and associated with significant improvements in glycated hemoglobin (HbA1c), low rates of hypoglycemia and limited weight gain. The aim of this prospective (4 months) observational study was to evaluate the safety and efficacy of a basal plus insulin regimen with glargine and glulisine once daily at main meal, in patients with Т2DМ in daily practice in Egypt.
Method: Adult patients with Т2DМ with HbAIc >7% while on combined Oral Anti-Diabetic Treatment (OAT) or basal insulin with or without OAT, whose physician had decided to switch to basal-plus regimen with glargine (one injection) and glulisine (one injection) were included in the study.
Efficacy of the basal plus regimen was assessed by comparing the mean HbAlc % and Post Prandial Blood Glucose (PPBG) level at baseline to that after 4 months of treatment. Safety of the basal plus regimen was assessed by tracking the occurrence of hypoglycemic events and other adverse events.
Results: A total of 308 patients with Т2DМ were included. HbA1c % showed a statistically significant mean reduction of 1.29x1.39% (p<0.001), and a reduction in the PPBG level was observed with a mean reduction of 70.28a-67.24mg/dl (p=0.467) after 4 months of treatment. Hypoglycemic events were reported in 6.82% of the study patients.
Coлe/usi0ns: Basal treatment with glargine once daily plus glulisine at main meal showed to be highly beneficial in glycemic control in Т2DМ patients in real life practice for duration of 4 months with a significant dropped in HbAlc values. It is indicated as a promising strategy in T2DМ management in daily practice in Egypt.
