This review explores the multifaceted landscape of med-ication adherence among older adults with multiple chronic conditions, emphasizing the roles of healthcare professionals in supporting adherence. Factors influencing adherence, includ-ing demographic, clinical, and medication-related variables, are examined, highlighting both positive and negative associ-ations with adherence. Positive factors such as health literacy and disease-specific knowledge underscore the importance of patient education and empowerment, while challenges like polypharmacy and medication regimen complexity underscore the need for simplified treatment plans and close monitoring. Pharmacists and nurses play integral roles in supporting ad-herence through motivational interviewing, patient education, and self-management support. Collaborative efforts in screen-ing, assessing, and addressing adherence issues demonstrate the importance of interdisciplinary care in optimizing patient outcomes. Effective communication and information exchange among healthcare professionals are essential for coordinating patient care and ensuring continuity across settings. Tailored interventions that engage patients and utilize technology, such as secure web platforms and patient diaries, show promise in enhancing adherence and promoting long-term monitoring. By recognizing and addressing the diverse factors influencing ad-herence, healthcare professionals can optimize treatment out-comes and improve the overall quality of care for this vulner-able population.
