Non-Surgical Management for Class II Obesity in Adult People (Systematic Review), HANY F.M. ELSISI, EBTEHAL A. AFYA, AWNY F. RAHMY and OSAMA FEKRY
Abstract
Background and Aim of Study: Physical therapists do effort to determine the best interventions suitable for treatment of class II obesity. This systematic review is a compilation of like studies to address a specific clinical question using a detailed, for the purpose of summarizing, appraising, and communicating the results and implications of all the research available. Medical libraries were searched using keywords of different types of interventions "behavioral therapy", "physical activity", "dietary therapy" and "pharmacotherapy".
Methods: This study was registered in Cairo University, Faculty of Physical Therapy to review database over the selected topic “non-surgical management for class II obesity in adult people”.
Patients and Methods: Data search, Formation of results database, Articles selection, The Appraisal process, Expanding search results and Data Extraction and Synthesis.
Patients participating in this review varied in number and age in each clinical trial. Ages varies from 20 up to 60 years old. And numbers also varies from 3 to several hundred in different controlled trials.
Results: Significance of meal replacement, "low fat diet intervention", VLCD, "high protein diet" and using low carbohydrate diet, physical interventions, behavioral interven-tions, pharmacological interventions and combinations of interventions in weight loss are confirmed by many random-ized clinical trials. The best effect and also the lowest effect of different used methods is determined.
Conclusion and Recommendations: From this review a series of clinical and research recommendations were driven to optimize the physical therapy adjacent management. VLCD and high protein diet are the best methods of diet to achieve weight managament. Sibutramine and orlistat are the best medications in weight management in long term, and sibutraime is slightly better in short term. The best way for management of obesity is the combination of dietary and behavioral interventions.
