Serum Concentration of Free Testosterone and Insulin Resistance in Early Miscarriage in Nulliparous Women with and without Polycystic Ovarian Syndrome, MOHAMED A. ELNAGGAR, ASHRAF M. KANSOUH and AHMED E. EL-SHEIK
Abstract
Backgroud: Polycystic Ovarian Syndrome (PCOS) is one of the most common endocrine disorder affects 5-10% of women of reproductive age. Hyperandrogenism, chronic anovulation, and infertility are the main features of this heterogeneous condition in the absence of specific diseases of the ovaries, adrenals and pituitary.
Objective: The present study was designed to investigate serum free testosterone levels and insulin resistance associated with miscarriage in patient with and without PCOS.
Methods: The study was done at Department of Obstetrics and Gynecology at Benha and Shibeen El-Kom Teaching Hospital, from October 2015 to September 2016. It included 100 cases of early miscarried women, who were divided in two groups 50 women with PCOS (Group A); and 50 women without PCOS (Group B). All women were submitted to full history taking, thorough clinical examination and laboratory investigations; measurement of serum free testosterone, fasting plasma glucose, fasting serum insulin and estimation of insulin resistance.
Results: The results of the present study showed that, there was statistically highly significant difference between both groups as regard to serum concentration of free testoster-one; (Group (A); mean: 28.08±3.46pg/mL, Group (B); mean: 17.20±2.56pg/mL, p-value <0.001), there was statistically non-significant difference between both groups as regard to fasting plasma glucose (Group (A); mean: 87.93±2.33mg/dl, Group (B); mean: 88.53±2.15mg/dl, p-value 0.15), there was statistically highly significant difference between both groups as regard to serum fasting insulin; (Group (A); mean: 32.74± 2.46mIU/mL, Group (B); mean: 10.56±0.85mIU/mL, p-value <0.001) and there was statistically highly significant difference between both groups as regard to insulin resistance; (Group (A); mean: 0.289±0.002, Group (B); mean: 0.337±0.004, p-value <0.001).
