Comparative Study between Combined Use of Radiofrequency with Transarterial Chemoembolization and Use of Radiofrequency Alone in Treatment of Hepatocellular Carcinoma, IBRAHIM A. NEGM, SAMY ZAKY, AMR M. ZAYED, ISLAM AMMAR and HASSAN E. ABDALLA
Abstract
Background: Purpose: То evaluate the safety and the efficacy of Transcatheter Arterial Chemoembolization (TALE) and Radiofrequency Ablation (RFA) compared to use (RFA) alone in treatment of hepatocellular carcinoma.
Patients and Methods: One hundred patients with medium size single hepatic focal lesion (3.1-5em), no vascular invasion, and no extra hepatic metastaseswere randomly divided into two groups; group I (50 patients) was treated with RFA alone; group П (50 patients) was treated with TACE and RFA combination. Overall survival, recurrence-free survival, and adverse effects were studied.
Results: Complete ablation at 1 month was achieved in (92%) with group II (TALE-RFA) which was significantly better than that observed with group I (RFA) (76%о), p-value= 0.02. Patients in group П showed significant better recurrence free survival rate compared with those in group I; at 12 months (74.4% vs. 625%p ).08) and at 18 months (78.2% vs. 51.4%, p=0.03). Also, the survival rate was significantly better in group П (TACE-RFA) than that of group I (RFA) at both 12 months (92% vs. 80% p=0.07) and 18 months (80% vs. 70% p=О.04). There were no treatment-related deaths and there was insignificant difference between the two groups as regard minor or major complication.
Conclusions: The combination of TALE and RFA in the treatment of medium-sized HCC (3.1-5) cm was found to be more effective than that of RFA alone, where it provides better local tumor control and better overall survival without significant difference in minor or major complications.
