Abstract
Background: The selection of donors for adult-to-adult Living Donor Liver Transplantation (LDLT) is one of the most important aspects of this procedure. Criteria for donor selection are evolving as experience with the procedure increases. However, the two fundamental purposes of the donor evaluation are to ensure that the donor operation may be performed safely, and the donor is able to yield a suitable graft for the recipient. Donor safety is the single most important clinical issue related to LDLT. It is very important to calculate the graft and remnant liver volumes before hepatic resection, in order to ensure adequate hepatic function and liver regeneration after surgery, both in recipients and donors.
Aim of the Study: The objective is to evaluate the effect of graft recipient weight ratio on the outcome of living donor liver transplantation.
Patients and Methods: This present prospective study had been conducted on 48 patients with HCV related liver cirrhosis who underwent liver transplantation at El-Mania! Hospital over period of 12 months duration, from 30/8/2013 to 30/812014.
Results: In the present study it has been found that when the donor GRWR was s0.8, 20% of recipient grafts were rejected, but when the donor GRWR was >0.8, rejection occurred in 16.3%. Regarding mortality, recipient modality dramatically rised to 60% when the donor GRWR was s0.8, but when the donor GRWR was >0.8, mortality occurred in 25.6% of recipients only.
Conclusion: Graft Recipient Weight Ratio (GRWR) is one of the risk factors that had a high impact on recipient mortality and graft rejection.
